You need the MysticTicket event item, which was distributed at Nintendo events in 2004-2005. With the ticket, talk to the sailor in Vermilion City to sail there. On the original cartridge, this event has long expired. The 2026 Switch re-release may offer a new distribution method.
Yes, both at level 70. Go up the exterior path for Ho-Oh, down the interior cave path for Lugia. They're standing encounters so save before each one. These are the only way to get Lugia and Ho-Oh in any Gen 3 game without trading from Colosseum or XD.
Islands One through Three unlock after you beat Blaine on Cinnabar Island. Bill takes you there as part of the story. Islands Four through Seven require beating the Elite Four first, then completing Celio's Ruby and Sapphire quest on One Island to get the Rainbow Pass.
You can physically reach all seven islands without it, but evolved Johto Pokemon only show up in the wild after you get the National Dex. That means 60 or more Pokedex entries plus completing Celio's quest. If you're only seeing Kanto species, that's why.
